Skilled Nursing Facilities (SNFs) are licensed health care facilities for patients who need short-term rehabilitation services or nursing care. SNFs are often referred to as nursing homes, although they have distinct differences.

Patients may go to a SNF for recovery after a hospitalization for a surgery, fall, stroke or accident.

SNFs offer round-the-clock nursing care and assistance with meals, personal hygiene and medications. SNF staff may:

  • Attend to post-operative wounds
  • Dispense and monitor IV medications
  • Work on physical therapy, speech therapy and occupational therapy
  • Assist in discharge planning, including referrals for home health visits, durable medical equipment, long-term care options, or hospice

What is the OSF Skilled Nursing Network?

The OSF Skilled Nursing Network is a select group of SNFs that meet a strict set of quality and safety standards set for by the Centers for Medicare & Medicaid Services (CMS) and OSF HealthCare.

SNFs are reviewed regularly to ensure they continue to meet the criteria.

The partnership between the SNF Network facilities and OSF HealthCare is strengthened by our close work together; it is a partnership dedicated to assisting you in achieving the best outcomes possible.

How is OSF HealthCare involved in the care of these patients?

Advanced Practice Providers (APPs) are the heart of the program.

These advanced practice registered nurses and physician assistants will visit you soon after you arrive at the SNF, and may visit you multiple times a week.

Your SNF practice doctor will also see you soon after you arrive and work closely with the APPs throughout your entire stay.

What is the benefit of a skilled nursing facility (SNF)?

The goal of a SNF is to:

  • Prevent patients from returning to the hospital for care that can be managed at the SNF
  • Provide care, including frequent therapy sessions, that will help you return to your original level of functioning
  • Assist you with a seamless transition from the hospital to a SNF and back into the home or other level of care
